10 YRS. OLD - 17 YRS. OLD
 

• Accomplished Certificate of Live Birth (4 copies ) - Typewritten

• Baptismal

• School Record (Form 137 - Elementary) - Certified copy

• Marriage Contract of Parents - Certified copy / Issued by LCR / NSO (If Married)

• Verification from NSO (Negative result) - 2nd Flr. Extension of NSO Pasig City Hall

• Cedula - Mother / Father - latest

• Acknowledgement of father / Admission of Paternity (If not married)

• Affidavit to use the surname of the father (AUSF) - if not married

• Affidavit of Legitimation

• Certificate of no Marriage (CENOMAR) issued by NSO - if legitimated child

Reminders:
All birth certificate & Marriage contracts for submission must be certified photocopy if issued in Manila, if issued outside Manila City submit SECPA from NSO.
All supporting documents are subject for verification. Do not submit "Fake Documents" to avoid the penalty of imprisonment.
Do not make any erasures in the preparation of Birth Certificate otherwise it shall be retuned to the registration for retype.
Be sure that you have completed all the requirements upon submission of your Birth Certificate.

  • Voter Affidavit - Certified by COMELEC

  • SSS (E-1)

  • Driver's License

  • Service Record / 201 File

• Passport

• Marriage Contract of Parents / Registrant - Certified copy / Issued by LCR / NSO (If Married)

• Verification from NSO (Negative Result) - 2nd Flr. Extension of NSO Pasig City Hall

• Cedula - Mohter / Father / Registrant - latest

• Acknowledgement of father / Admission of Paternity (If not married)

• Affidavit of Legitimation

• Certificate of no Marriage (CENOMAR) issued by NSO - if legitimated child

• Cedula - Mother / Father - latest 2005

• Acknowledgement of father / Admission of Paternity (If not married)

• Affidavit to use the surname of the Father (AUSF) - if not married

• Affidavit of Legitimation

• Certificate of Marriage (CENOMAR) issued by NSO - if legitimated child
